Timely filing is the window a payer allows between the date of service and when a claim must be received. The length of the window varies by payer and plan, and missing it usually means the claim cannot be paid at all.
The clock generally runs from the date of service, and it does not stop just because a claim was rejected or is being reworked. Proof of timely submission is often required to overturn a timely-filing denial.
